Summary

• move reviewed-apply review mutation, second-guardrail precheck, and request-input assembly into noname_apply.rs
• keep tauri_commands.rs focused on parameter intake, runtime/engine orchestration, and error propagation
• sync NoName architecture and planning docs to mark A2/A3 as phase-complete after the fifth refactor pass
