[COSMETIC] Add header rows to chart of accounts

The Chart of Accounts page (Finance -> CoA) should have header rows for CoA account types. Consider indenting per account hierarchy

Reports -> CoA has the correct formatting and also uses the include balances switch. Both the Financials screen and the Reports screen should use the same control to display the CoA grid.
