Wrong link to task

Hi again!

One of my colleague got the plugin working and he reported that the link to a task is wrong.

Steps to reproduce:

1. Open VSCode Extension
2. Choose a task
3. Click on the headline
4. Browser tab opens
5. Redirect to https://teamwork.{company}.com/#/home/activity

Our task link structure is: https://teamwork.eu.{company}.com/#/tasks/{task_id} (global unique task id's)

Just noticed that not the whole link is broken, just the "#" is missing

Instead of
https://teamx.eu.teamwork.com//tasks/xxxx

the link should be:
https://teamx.eu.teamwork.com/#/tasks/xxxx

Any hint for me where to fix it?
