Port-specific resolution

The repository's three-way sync mechanism applied the non-overlapping upstream edits directly. Overlaps were resolved in favor of the existing Claude/Herdr transport and safety adaptations while carrying over the upstream behavior changes.

In particular:

• Cursor-only Grok model slugs are not introduced into this Claude/Codex port; code-role defaults continue through the port's role/model policy.
• setup-pstack keeps this fork's existing per-profile model + optional effort configuration instead of importing upstream's global reasoning-budget preset.
• Herdr now validates effort values by worker kind: Claude accepts low|medium|high|xhigh|max; Codex accepts minimal|low|medium|high|xhigh.
• Existing Herdr worktree, transcript, driver, and operator-click merge semantics are retained.
