feat(server): pull request watch remembers the user's instructions - #179
A watch that runs for hours forgets how the user wanted it handled (which bots to trust, what not to touch, when to stop), because the only copy lives in the first message and scrolls out of the agent's context.
Repeating the user's standing instructions in every wake keeps each pass faithful to the original request without the user having to restate it.
Standing instructions for babysitting a pull request lived only in the conversation, so context compaction lost them and wakes never reminded the agent.

Medium Risk
Changes orchestrator watch mutation and wake dispatch semantics; incorrect instruction merge or stale-wake handling could drop or duplicate agent context, though coverage is extensive.
Overview
PR watches can now store standing instructions (e.g. ignore bot nits, ask before replying) so long babysit sessions do not lose the user’s intent after the first message scrolls away.
watch_pull_request accepts an optional instructions field (omit to keep current text, empty string to clear). The MCP handler and thread.pull-request.watch command persist instructions on ThreadPullRequestWatch. The orchestrator updates them via withWatchInstructions without resetting check/comment cursors when the user re-watches or edits instructions only; stale wake syncs are rejected if instructions changed mid-read.
Each PR watch wake message from pullRequestWatchMessage repeats the stored instructions. The linked-PR panel tooltip shows them when present; watchdog prompts and user docs tell agents to pass instructions when babysitting.
Contracts add validated instruction schemas (max 1000 chars). Tests and an MCP eval guard that babysit requests record instructions on the watch call.

Pull-request watches now accept optional standing instructions. The system stores and preserves them across watch updates, includes them in watch messages, and displays or requests them through related interfaces.
The watch contract adds validated optional instructions, with empty input permitted for clearing. The MCP tool accepts the field and forwards supplied instructions in watch commands.

Watch updates preserve instructions when omitted, replace them when supplied, and clear them for empty input. Watch state carries instructions forward, and equality checks include them.

Watch messages include supplied instructions. The prompt, tooltip, documentation, and evaluation case reflect how callers provide and view them.

A watch wake can repeat outdated instructions if the user changes them during polling. The exposure is bounded to that wake; merging requires awareness of this remaining race.

Explanation
The title "feat(server): pull request watch remembers the user's instructions" matches the main change. The diff adds optional standing instructions to the pull-request watch across contracts, the orchestrator, the watch message, the MCP tool, and the web watch prompt, so the watch keeps and repeats the user's instructions on each wake. It is a conventional commit title in plain language.
Full details: Description check
Explanation
The description states the problem: a long-running watch loses the user's handling rules (which bots to trust, what not to touch, when to stop) once the first message scrolls out of the agent's context. It also states the change: repeating the standing instructions on every wake. However, the template's required sections (Change, Scope and approval, Verification) are absent. There is no explanation of the implementation, no link to an approved issue or discussion, and no verification steps or test results. The description does not mention the new instructions field on thread.pull-request.watch, the 1,000-character limit, or the clear-with-empty-string behavior.
Resolution
Add the missing template sections: (1) Change: summarize the implementation. Cover the new optional instructions field on the watch command and watch state, the rule that omitting it keeps current instructions and an empty string clears them, the 1,000-character limit, and the update to the watch_pull_request tool and watch prompt. (2) Scope and approval: link the triaged issue or the maintainer-approved discussion, or explain why this change qualifies without one. (3) Verification: list the focused tests run (for example, the new runtimeLayer, pullRequestWatch, handlers, and contracts tests and the eval case) with observed results, and state anything not checked. (4) Note the model and harness used, if an agent did the work.

Require the reviewer clause in the same watch call.
The case checks only the CodeRabbit clause. Its two calls entries are graded independently, so the proposed second entry can pass with a separate watch_pull_request call. Limit the case to one watch call and require both substrings in that call.
With maxCalls set to one watch_pull_request call, both calls entries must match that single call. The ~reviewer substring is a heuristic. It will not match a paraphrase such as "human review" without the word "reviewer".

cap watch_pull_request at one call to prevent separate invocations from

The wake is written before the host read, so instructions changed during that read would reach the agent stale.
