fix(server): free finished delegated Codex threads early - #189
Every Codex thread runs its own copy of each configured MCP server, and a finished thread kept those processes for about an hour: 30 minutes of T3 idle time, then another fixed 30 minutes inside Codex after thread/unsubscribe.
Delegated tasks create one Codex thread each and are almost never resumed, so a parent that fans out work piled up dozens of idle MCP processes (76 under one app-server in about 3 hours) on a machine that was already swapping.
thread/archive is the only Codex call that stops a thread's MCP processes right away, and unarchiving immediately keeps the thread visible in Codex's own UIs and resumable as before, so delegated children can be released after a short grace period without losing anything.
Nothing showed why a thread stayed loaded or whether a release failed, so the leak was only visible after the machine was already swapping; unload attempts now report their outcome so the fix can be confirmed and regressions caught on a dashboard.
Medium Risk
Changes provider thread unload timing and Codex archive/unarchive protocol paths; bugs could leave threads stuck loaded or interrupt work incorrectly, though behavior is scoped mainly to delegated children and is heavily tested.
Overview Delegated (subagent) Codex threads now idle out after 2 minutes instead of the default session idle window, and unload in eager mode so native MCP processes are torn down immediately rather than waiting on Codex’s long post-unsubscribe delay.
Codex unloadThread gains an eager flag: normal unloads still use thread/unsubscribe; eager unloads call thread/archive, fall back to unsubscribe if archive fails, interrupt running command items locally (because archive does not emit thread/closed), then thread/unarchive so the thread stays resumable in Codex UIs. Command interruption logic is extracted into a shared helper used by both thread/closed and eager unload.
Provider session manager tracks whether an idle unload is for a delegated child, passes eager into the adapter, classifies each attempt as unloaded / deferred / skipped / failed, and increments t3_provider_thread_unloads_total (mode + outcome) for observability. Tests cover eager delegated timing, deferred retries when background work blocks unload, archive success/failure replay paths, and command interruption on eager unload.

Walkthrough
Delegated child threads now use a shorter idle timeout and eager unloading. ProviderSessionManagerV2 records unload attempt outcomes. Codex eager unloading archives and unarchives the native thread, and interrupts running commands associated with the closed thread.

Summary
Delegated-child timeout and unload tracking packages/provider-core/src/server/ProviderAdapter.ts, apps/server/src/orchestration-v2/ProviderSessionManager.ts, apps/server/src/orchestration-v2/ProviderSessionManager.test.ts, apps/server/src/observability/Metrics.ts
ProviderAdapterV2SessionRuntime.unloadThread accepts an optional eager flag. ProviderSessionManagerV2 uses the shorter of the two-minute delegated-child timeout and configured session idle timeout, passes eager: true for delegated children, and records unload outcomes in spans and metrics. Tests cover unload timing, deferrals, failures, and metrics.
Non-eager unloads unsubscribe and return. Eager unloads archive and then unarchive the native thread, while interrupting running commands associated with the closed thread. Replay tests cover successful archive and archive failure.
Mergeable with owner awareness: unload telemetry can misclassify stale attempts, and an unarchive failure can leave a child archived in Codex until it is resumed.

The new eager unload relies on hasPendingBackgroundWorkForThread to protect native descendants, but Codex's supplied probe finds descendants only through active or settled root turn contexts. A completed child root with a still-running native grandchild may no longer have such a context, making eager archive possible while descendant work remains.
Archiving the child may interrupt the grandchild and lose its expected completion.
This can occur when: A child turn settles, its native grandchild continues working, and the child's two-minute unload fires.

and ensure the eager unloadThread call for pending.delegatedChild remains
