Documents the new naming and metric introspection in templates/code/go/README.md and refreshes all golden test outputs. Adds generation-error fixtures for identifier clashes exposed by shorter names (e.g. a metric vs *.duration.attr, a span vs *.run.name, enum member vs attribute key).

This is a breaking change for anyone already using previously generated packages.
