Move the accumulated Unreleased batch under 1.12.0 (2026-07-08), bump __version__, and drop a stale next-release note from the versioning preamble. Minor bump: the batch adds features (history filters, bulk approve, auto-approve by rule, notification settings, scoped review authority) plus migrations 0038-0041, with no breaking changes.
