Remove inline Phase 2-5 bullet list from CLAUDE.md to avoid patrol generating duplicate issues. docs/15-milestones.md is the single authoritative milestone source for bullet-to-issue conversion.

Removes inline Phase 2-5 bullet list from CLAUDE.md. Having two parallel roadmaps caused patrol to generate duplicate issues — once from CLAUDE.md bullets and once from the milestone-status pipeline.
docs/15-milestones.md is the single authoritative milestone source.
