Update the generated run_tier block so fast-tier runs stop after their first failed tier.

Stop before a 501st input mutates the PSBT. Return a retryable confirmed-input reason when confirmed-only selection cannot fund within the cap, while preserving the existing range refusal when unconfirmed inputs are allowed.
Both the confirmed-only and unconfirmed paths now stop at the input cap with the retryable NO_CONFIRMED_UTXO reason instead of a range error.
Keep NO_CONFIRMED_UTXO specific to confirmed-only selection while stopping every path before a 501st input is added. Unconfirmed-enabled selection continues to use the established range refusal when largest-first selection cannot fund within the cap.
