Add numbered continue menu

Summary

• replace the TTY agt continue shortcut prompt with a numbered next-step menu
• add a feedback/rework path with categorized templates before free-form input
• keep existing keyboard shortcuts as aliases while making numbers the primary UI
• keep non-TTY/script behavior unchanged
