HDX Interface Issue (minor): Warning for missing CoC Code loses file path on refresh

1. From https://hdx-sandbox.abtsites.com/module/lsa/uploads/create, Browse to Find File -> Select File
2. Select Upload Type
3. Add Upload Name
4. Do not select CoC
5. Click upload
6. Error message presents as expected but the file path is also dropped

I've noticed this behavior as well. Very minor, yet still mildly frustrating.
