check_pending_intents: alert on ledger intents pending >= 10m - PaymentUncertainMonitor: alert when payment_uncertain count is stuck >= 15m - wire both into the APScheduler runner (1m and 5m jobs)
charging twice (sweep+execute x2) debits Nomba exactly once - a crash AT the Nomba call leaves the intent durably pending (RPO=0)
