Follow-up to #538. Consulted agents.md sections: Maintaining This File (placement, timeless wording, shed-to-HISTORY, cite-by-symbol), What is pi-free, Architecture at a Glance, Provider Categories, Recurring Defect Shapes, Testing, Release Workflow (changelog gate).
Lesson recorded: defect shape 10 (dual pi-ai copies) with the three proven don'ts (peer floor in lockstep; no devDependency; overrides can't target peers).
Lean pass (net -8 lines, dedup only — no live rules removed):
HISTORY.md: seeded with the #539 incident narrative per its shedding discipline (live rule stays in agents.md). Also added the missing [Unreleased] Fixed changelog entry for #538.
