Why

Logto's audit log for the Archgate CLI client shows 1,158 failed refresh_token exchanges against 49 successful ones since 2026-10-04, all from archgate-cli/0.58.0–0.60.1 on Windows. Nearly all fail with invalid_grant: refresh token not found. They arrive in runs of hundreds per machine across several days, often seconds apart.

Two CLI behaviours produce that:

  1. A refused session stays stored. On invalid_grant the CLI reports signed out but keeps the record. Every later archgate credential get (git calls it for each plugin fetch) and every CLI command sends the same dead refresh token again, until the user runs archgate login.
  2. Concurrent renewals replay the refresh token. Git starts the credential helper several times per operation, often in parallel. Each process that found the lapsed session exchanged the same refresh token. Logto rotates it on use, and a second use of a consumed token (refresh token already used) revokes the whole grant, so every token in it then reads as "not found". The previous recovery (re-read the store after a refusal) ran only after the replay had already revoked the grant.

What changes

  • src/helpers/refresh-lock.ts (new): withRefreshLock(task).
    • Each renewal files a uniquely named entry, ~/.archgate/refresh-lock/<zero-padded ms>-<uuid>, and the oldest live entry holds the lock.
    • Names never repeat. Releasing your own entry or removing an abandoned one can never remove another holder's, and no shared path is ever renamed or reclaimed.
    • Before reading the clock, a caller creates pending-<uuid>, and removes it once its entry is filed. Nobody holds the lock while a live marker exists (the bakery algorithm), so a caller paused between its timestamp and its entry is always visible.
    • A caller decides only once the clock has passed its own timestamp, so anyone who marks itself afterwards takes a strictly later timestamp and sorts after it.
    • Waiters poll every 100 ms and refresh their entry's mtime each time. An entry untouched for 30 s counts as abandoned and is removed. 30 s exceeds the 15 s token request plus the store write and read-back.
    • If the lock cannot be had (directory not writable or readable, or still held after 40 s), the caller withdraws its entry and the task runs anyway, so a git operation is never failed by the lock.
  • src/helpers/credential-store.ts:
    • Renewal runs under the lock and re-reads the session once the lock is held. A waiter normally finds a fresh token set saved by the holder and does no exchange.
    • When the platform refuses the refresh token, the store is re-read up to 4 times, 250 ms apart, for a newer session saved by a process that renewed without the lock. Only if none appears is the stored session removed, and only if the record still holds the refused refresh token.
    • Failures that are not invalid_grant (service unreachable, 5xx) leave the session untouched.
  • package.json, bun.lock: zod 4.6.6, for GHSA-mrhx-c7xc-m3w5, which fails the dependency audit.

Tests

  • tests/helpers/refresh-lock.test.ts, all in a mkdtemp sandbox:
    • an entry is filed for the task and removed afterwards, including when the task throws
    • six concurrent renewals with an abandoned entry pre-seeded never overlap (peak 1)
    • an abandoned entry is removed; a live older holder is waited on until the deadline; a newer entry does not block
    • an entry filed in the same millisecond that sorts first makes the earlier caller yield
    • a caller that marked itself but has not filed yet holds others back, then runs first once filed, with no overlap
    • an abandoned marker is removed, and a failed entry write runs the task without the lock
    • non-entry files are ignored
    • unwritable or unreadable directory, an entry vanishing mid-check, and a waiter whose own entry disappears
    • the default directory under HOME
  • tests/helpers/credential-store-renewal.test.ts:
    • renewal runs inside the lock
    • a waiter uses the holder's token set without exchanging
    • a session gone once the lock is held reports signed out
    • a refused session is removed; a session saved elsewhere after the refusal is used; an empty or unreadable record is left alone
    • a failed removal still reports signed out
    • a non-invalid_grant failure keeps the session
  • Coverage for both changed modules: 100% lines and functions.

Checks run locally

  • bun run validate: pass (2926 tests, ADR check 28/28, knip, build check). The --version startup-budget check missed under load once and passes on its own.
  • Reviewer: no remaining ADR violations; its findings (un-awaited rejects, precomputed booleans, unconditional release, the write-failure leak) are fixed in the second commit.

Not covered

  • A holder that runs past 30 s without finishing has its entry treated as abandoned, so a waiter can start alongside it. The token request alone times out at 15 s.
  • Entry order relies on the system clock. A clock set backwards while renewals are queued can let a later entry sort first.
  • Sessions already revoked on users' machines recover on their next CLI run: the first refusal removes the record, and archgate login is then required once.

📝 Walkthrough

Expired-session renewal now runs under withRefreshLock. Renewal re-reads stored credentials and reuses a session if another process has already renewed it. After a refused refresh, the code removes the stored session only when its refresh token still matches. The change adds tests for lock behavior, renewal outcomes, and session removal. It also updates the zod development dependency from 4.6.5 to 4.6.6.

Full details: Title check

Explanation

The title "fix(auth): serialize session renewal and drop refused sessions" matches the main changes. The PR adds a filesystem refresh lock that serializes renewals across processes (src/helpers/refresh-lock.ts, credential-store.ts changes). It also removes a stored session after a refused refresh token, which is the second part of the title. The title is specific and concise, and it follows the conventional commit form. The zod bump and test additions are minor by comparison and need no mention.

Full details: Description check

Explanation

The description is closely related to the changeset. It explains the cause (refused sessions kept in storage, and concurrent refresh-token replays). It lists the new src/helpers/refresh-lock.ts module and the credential-store.ts changes, the zod 4.6.6 bump in package.json and bun.lock, and the new tests. It also states the known limits. The level of detail is high, and every part maps to the changed files.

Git runs the credential helper several times per operation, often in
parallel. Each process that found a lapsed session exchanged the same
refresh token; the platform rotates it on use and revokes the whole
grant when an old one is replayed. A refused session also stayed in the
store, so every later lookup sent the dead token again.

- renewal runs under a lock file in ~/.archgate (O_EXCL create, taken
  over after 30s, renews without it if the wait runs out) and re-reads
  the session once the lock is held
- a refresh token the platform refuses is removed from the store, unless
  a sign-in has replaced that record meanwhile

Signed-off-by: Rhuan Barreto <rhuan@barreto.work>
The lock file carries a per-run token. Release and stale takeover check it, so a holder that outlives the stale window cannot delete a waiter's lock, and a lock file whose token could not be written is removed at once.

SessionExpiredError, it performs a bounded sequence of delayed readSession()
checks before discarding the session or rethrowing. If a reread finds a

both token comparisons instead of using readToken, which converts read errors to

other read errors leave the lock unreclaimed.

removing a replacement lock after a race. Leave ordinary lock acquisition

No recursion occurs through loadCredentials.

The plugin-host lookup can invoke archgate credential get, but that command calls resolveAccessToken(). resolveAccessToken() reads only the AUTH_HOST session, so it does not call loadCredentials() or perform another PLUGINS_HOST lookup. The registered helper is scoped to PLUGINS_HOST, so the AUTH_HOST read does not re-enter this helper.

Release and stale-lock removal run under a short-lived guard file, so neither can unlink a lock that replaced the one it checked, and an unreadable token never counts as a match. A refused renewal re-reads the store briefly before discarding the session, in case a process that renewed without the lock saved a newer one.

Resolves GHSA-mrhx-c7xc-m3w5, which fails the dependency audit in CI.

Signed-off-by: Rhuan Barreto <rhuan@barreto.work>
Each guard carries a token and is removed only while it still holds it. An abandoned guard is reclaimed by renaming it to a unique name; if the renamed file turns out to be fresh, another reclaimer replaced the stale one in between, so it is linked back and this process yields.

Signed-off-by: Rhuan Barreto <rhuan@barreto.work>
Each renewal files ~/.archgate/refresh-lock/<timestamp>-<uuid>; the oldest live entry holds the lock. Names never repeat, so removing an abandoned entry or releasing one's own can never remove another holder's, and no takeover ever exposes a held path. A caller decides only once the clock has passed its own timestamp, so an entry filed in the same millisecond is already listed. Waiters refresh their entry while polling, so only entries whose process stopped count as abandoned.

Signed-off-by: Rhuan Barreto <rhuan@barreto.work>
A caller creates pending-<uuid> before reading the clock and removes it once its entry is filed, and nobody holds the lock while a live marker exists. A caller paused between its timestamp and its entry is therefore always visible, and a caller that marks itself after a decision takes a strictly later timestamp.

Signed-off-by: Rhuan Barreto <rhuan@barreto.work>
A marker that disappears between the directory listing and its age check usually means its entry was just filed after the listing was taken. The lock check now answers not-yet and the next poll rescans, instead of deciding from the stale listing.

🟡 Minor · Propagate the credential-helper timeout from renew to… · credential-store.ts:330-331

src/helpers/credential-store.ts:330-331
🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Propagate the credential-helper timeout from renew to resolveWithStatus.

renew calls readSession() and drops timedOut. If the helper is killed after 3 s on this re-read, session is null and renew returns null. Line 313 then returns timedOut: false.

loadCredentials then runs the legacy gitCredentialFill(PLUGINS_HOST) lookup. That adds a second timeout wait. The comment at Lines 452-453 says to skip this lookup after a timeout. A signed-in user whose helper is slow can also fall through to legacy credentials.

Return the timeout status from renew, for example { credentials, timedOut }. Then use it in resolveWithStatus.

Sketch
-async function renew(): Promise<StoredCredentials | null> {
-  const { session } = await readSession();
-  if (!session) return null;
+async function renew(): Promise<{
+  credentials: StoredCredentials | null;
+  timedOut: boolean;
+}> {
+  const { session, timedOut } = await readSession();
+  if (!session) return { credentials: null, timedOut };

Wrap the other return statements in renew as { credentials, timedOut: false }. In resolveWithStatus, return await withRefreshLock(renew) directly.

deviceFlowAuth.refreshAccessToken throws SessionExpiredError only when the parsed OAuth error is exactly invalid_grant. Other OAuth errors and bare HTTP 400/401 responses throw UserError, so discardSession does not remove the session for those cases.

removeEntry ignores only ENOENT and logs any other failure, and an abandoned entry that cannot be deleted keeps counting as a holder. touchEntry withdraws only when the entry is gone; other failures are logged, and a waiter whose refreshes keep failing withdraws after half of staleMs, before others can reap it. renew reports a helper timeout on its re-read, so loadCredentials skips the legacy lookup.

The outside-diff finding on credential-store.ts (helper timeout dropped in renew) is applied in ff6a4f3: renew returns { credentials, timedOut }, and resolveWithStatus returns it directly, so loadCredentials skips the legacy lookup after a timeout. Test: "skips the legacy lookup when the re-read inside the lock times out".

🤖 Addressed by Claude Code

A waiter sleeps at most until half of staleMs has passed since its last successful refresh and checks that window before trying to acquire, so a poll interval at or above staleMs can neither let a healthy waiter go stale nor delay withdrawal after failed refreshes. Heartbeat tests move to refresh-lock-heartbeat.test.ts.

A waiter refreshes once a quarter of staleMs has passed and withdraws only after half passes without a successful refresh, so a timer that fires late still refreshes in time. After a failed refresh the next wake-up is the withdrawal point rather than a 1 ms spin.

The healthy-heartbeat and late-timer tests use a 400 ms stale window and release the holder after 500 ms, so parallel test scheduling cannot push a refresh past the withdrawal point. Both assert the holder's entry is gone when the task runs, which a waiter that withdrew early cannot satisfy.

You can configure the bot's behavior through a pull request comment
using the `!simple-release/set-options` command.

### Command Format

````md
!simple-release/set-options
```json
{
  "bump": {}
}
```
````

### Useful Parameters

The options shape the pull request generation, so the `bump` ones matter
here. Release-time options like `publish` belong in the config file.

| Parameter | Type | Description |
|-----------|------|-------------|
| `version` | `string` | Force set specific version |
| `as` | `'major' \| 'minor' \| 'patch' \| 'prerelease'` | Release type.
`prerelease` keeps the type derived from the commits and makes the
version a prerelease |
| `prerelease` | `string` | Pre-release identifier (e.g., "alpha",
"beta") |
| `firstRelease` | `boolean` | Whether this is the first release |
| `skip` | `boolean` | Skip version bump |
| `byProject` | `Record<string, object>` | Per-project bump options for
monorepos |

### Usage Examples

#### Force specific version

````md
!simple-release/set-options
```json
{
  "bump": {
    "version": "2.0.0"
  }
}
```
````

#### Force major bump

````md
!simple-release/set-options
```json
{
  "bump": {
    "as": "major"
  }
}
```
````

#### Create alpha pre-release

````md
!simple-release/set-options
```json
{
  "bump": {
    "prerelease": "alpha"
  }
}
```
````

### Custom Changelog Preamble

You can add custom markdown to the top of the changelog (right after the
version header) using the `!simple-release/set-preamble` command. The
markdown after the command line becomes the preamble. Start its headings
at `###` — the level of the generated changelog sections like `###
Features`.

```md
!simple-release/set-preamble
### What's new?
- The website was completely redesigned
- The new API gives you awesome possibilities
```

In a monorepo, pass the full package name after the command to target a
single package's changelog. Wrap the name in backticks so GitHub keeps
it as text instead of a mention:

```md
!simple-release/set-preamble `@your-org/core`
### Core changes
- New plugin system
```

Use one comment per package, plus one without a name for the whole
release.

### Access Restrictions

The commands can only be used by users with permissions:
- repository owner
- organization member
- collaborator

### Notes

- The last comment with `!simple-release/set-options` command takes
priority
- The last `!simple-release/set-preamble` comment per package takes
priority
- JSON must be valid, otherwise the `set-options` command will be
ignored
- Parameters apply only to the current release execution
- The commands can be updated by editing the comment or adding a new one
