Fixes cold-start black screen by documenting v4l2loopback exclusive_caps=0 as a hard requirement (root cause was exclusive_caps=1 making the scrcpy-panel device invisible to Qt Multimedia at process start). Slims the PhoneDisplay state machine and diagnostic logging now that the root cause is understood; recovery timers kept as safety nets but with tighter intervals for a ~1s cold-start attach.
