Fix size followup intent gates

• Require operator confirmation before applying measured size-budget caps, including explicit hard-cap parse paths.
• Preserve heuristic measured-size follow-up detection when the structured note parser wins but misses that field.
• Add focused regression coverage for unconfirmed hard caps and merged follow-up parsing.
