0.24.0: platform-aware rules block, auto-detour fix, Real Numbers refresh

Mirrors pm 0.24.0 (cfdude/pm#72).

• Changelog page: new 0.24.0 entry — platform-aware rules block, per-platform command form, the test-suite split, the pre-commit ran-vs-declared guard, the auto-detour fix (#65/#68) including the first-line/shell-source subtlety, and the migration note.
• Introduction → Real Numbers: recomputed mechanically, not estimated — 282 tests (was 250), ~2,900 lines of engine code (was ~2,700). Releases already correct at 38.
• Introduction → platform Note: updated to say the rules block is now platform-aware groundwork, while being explicit that no second platform's commands or hooks ship yet. Deliberately not overstated.
• /commands/upgrade: the refresh step is no longer described as CLAUDE.md-only, and notes that an older block's delimiter wording is migrated in place (detection keys on a stable prefix, so a pre-0.24.0 block is refreshed rather than duplicated). Migration step mentions the new platform field.

Left alone on purpose: pages that say "the CLAUDE.md rules block" as shorthand (lane-routing, review-mode, tracker, external-trackers, quickstart). That is still accurate for Claude Code, which is every shipping user today, and churning them would add noise without helping a reader. The one historical conductor.test.mjs reference in the 0.18.0 changelog entry is also unchanged — it describes what shipped then.
