The refactor introduces three defects in persisted-event compatibility and pairing enforcement that should be fixed before merging.
Independent impact: High. These changes govern migration workers that overwrite column values and authorize removal of their encrypted source. Losing a recorded forced-pass obligation can leave stale plaintext undetected before that source is dropped; other defects can unnecessarily re-encrypt entire tables or misdirect resumed library migrations.

Review feedback audit at The review at d9f1c05 raised three findings. All three threads are resolved, and each fix is in the code at head:
One leftover case is the safe one. Suppose an older release paused a forced pass and then resumed it without This audit came from reading the code. I did not run the tests. Generated by Claude Code |

Comment-only review at c6b02894. I found no blocking defects. Four items should be fixed and four are nits. I read the code but did not run the tests.
The branch has merge conflicts with main and needs a rebase before it can merge.
What holds
- The obligation state machine is correct over the legacy histories that the earlier review raised. The fixes in
c6b02894coveracknowledgedWith: 'force', legacy forward completions, and the twin recorded by the engine. - Resume carries the committed cursor and counts. A mode mismatch starts a fresh pass, and
countPending(after)prevents a double count. - Error events hold only
errorNameand safe details. SQL takes parameters or quotes identifiers. I found no new path where plaintext reaches events, output or logs. - The
minorbump is correct, because the removed exports were never published. dropgates on phase, obligations, the recorded twin and live coverage. The engine makes its own pass plan, so a library caller cannot skip a forced pass.- The skill text matches the code, except for the nit below.
Should-fix
- A forward migration is not superseded after a reverse round-trip. See
column-migration.ts:174. The superseded rule applies only whendirection === 'reverse'. Scenario: forwardemailtoemail_encryptedand drop, then reverse and drop, then encryptemailagain.ColumnMigration.load({ direction: 'forward' })still reportsphase: 'dropped'.confirmDualWrites()then takes thephase !== nullbranch at about line 539 and records nothing, so the new migration gets nodual_writingevent and its obligation scope stays on the old events. Make the rule symmetric, or letconfirmDualWritesrecord a new start when!this.open. - The new
decrypt statusflags have no positive tests.forward-openandforced-pass-owed(status.ts:257-259) are the headline additions. The only test asserts thatforced-pass-owedis absent, atdecrypt-lifecycle.live.test.ts:253. Add live cases that make each flag appear. In the forced case, also assert that--confirm-plaintext-currentis refused. - Two backfill tests do not test what their names say.
- At
decrypt-backfill.test.ts:395, the test says the waiver "may refuse it". But the fakeconfirmDualWritesinfake-migration.ts:99-108never throws and ignoreswaiver. - At line 410, the test names "a finished earlier column migration", but it uses an empty history.
- Make the fake throw
ColumnMigrationError('waiver-refused')when the obligation is not waivable. Then give the second test a real superseded history, or rename it.
- At
decrypt statusreports a finished reversal as drift. IngoneRows(status.ts:139-156), take a reverse migration whose drop was applied, where the plaintext column was later encrypted again under a different name. That history now reads as superseded withphasenull. The row shows—andencrypted-col-missinginstead ofencrypted-dropped. Skip rows withmigration.phase === null, or show them as finished history.
Nits
- The registry (
registry.ts:704) and thestash-cliskill say the waiver "is refused from then on" after a forced pass starts. After that pass completes, the waiver is a silent no-op. Say "refused while a started forced pass is owed". resolve-pair.ts:111-114adds "Drop --plaintext-column to continue with the recorded column" to everyColumnMigrationError. That includes the case where the twin equals the column and nothing is recorded. Add the hint only for a mismatch.- The test "the engine records the twin itself, so a paused run is not resumed into another" aborts before any chunk runs. So it proves the twin is recorded and a mismatch is refused, but not the resume path. Abort in
onProgressafter chunk 0, and fix the comment. - The changeset does not mention that
runBackfill, published in 1.0.0, now throwsColumnMigrationError('twin-mismatch')whenencryptedColumndiffers from the recorded twin. Add one sentence.
Already in the base, so not counted
- A run killed after
startedand before its first commit resumes from row 1, becauseresumeFromignores astartedevent'scursorValue. This costs more now, because the forward forced pass re-encrypts every row. - Two workers on the same column have no lock.
… into it
One module now owns the column-migration rules both directions follow:
which twin a column migration fills, whether it is the one in progress,
and the forced-pass obligation from ADR 0001. The engine asks it for the
pass mode and the resume point instead of taking `trackSkipped`,
`resumeFromError` and `canResume` from each caller.
- `ColumnMigration.load()` returns `{ phase, twin, obligations, blockedBy }`;
phase changes go through `confirmDualWrites({ waiver? })`,
`recordDropGenerated()` and `recordSourceRemoved()`. The rules are a pure
function of the event history (`deriveColumnMigration`, `planPassFor`),
covered by table tests.
- Every engine event records the pass mode. An open obligation forces the
run, and the result says why (`forcedBy`). A reverse pass over a twin that
already held values is owed a forced pass unless a waiver is recorded.
- Forward runs now resume after an `error` event, count their total from
the resume cursor, and finish an interrupted `--force` pass as a forced
pass.
- `error` events store `errorName` plus details each direction marks as
safe, never `err.message`.
- A run is refused while the other direction's column migration of the
pair is open, or when its twin contradicts the recorded one.
- `forceObligationOpen` and `ChunkedRunEvents` are removed (unreleased).

decrypt plan, status, backfill and drop now ask @cipherstash/migrate's ColumnMigration for the twin, phase, obligations and open counterpart instead of each deriving them. A live-Postgres scenario runs the four commands end to end and checks they name the same twin; it replaces the mocked status unit test. The backfill help is held to the constants the command enforces, and states that the waiver is refused once a forced pass has started. …he twin in the engine - A decrypt_dual_writing event recorded as acknowledgedWith: 'force' by the earlier decrypt CLI opens the up-front forced-pass obligation, as existingTwinValues does now. - A completion event without details.force, as the earlier forward backfill wrote, takes the mode of the pass started before it, so a completed legacy forced pass is not owed again. - runChunkedMigration records the twin on every event itself, so a direct caller that passes no baseDetails cannot resume a paused run into a different twin. - A forward column migration dropped and then reversed is superseded, like the reverse case, so encrypting the column again records a new dual_writing event instead of reading as 'dropped'. - `decrypt status` leaves out a finished reversal (drop applied, column migrated back) instead of flagging it encrypted-col-missing. - Live tests make forward-open and forced-pass-owed appear, and check the waiver is refused while a started forced pass is owed. - The backfill tests' fake ColumnMigration applies the real waiver rules, and the superseded-history test uses a superseded history. - resolve-pair suggests the recorded column only on a real mismatch. - The engine twin test pauses after a committed chunk and resumes it. - Help text, skills and changeset: the waiver is refused while a started forced pass is owed; runBackfill throws twin-mismatch. recordSourceRemoved wrote its dropped event without the twin, so a forward migration closed that way could not be superseded by the reverse migration of its twin, and re-encrypting the column later skipped its dual-write confirmation. The stash-cli skill no longer says concurrent encrypt backfill runners converge: nothing serializes them. Recommendation: 🟡 merge after changes
No finding blocks the merge on its own, but add four missing tests before merging: the forced-pass refusals in decrypt drop and recordDropGenerated(), the forward waiver refusal, and the ColumnMigration.load() fallback. The two follow-ups (engine detail overwrite, scalar details crash) and the optional type fixes can wait.

A caller's checkpointDetails or errorDetails was spread after the engine's twin, force and rowsSkipped, so it could overwrite the fields a resume reads. The engine's fields now go last. A cs_migrations row whose jsonb details is a scalar no longer throws in passForced. Adds the tests the review asked for: recordDropGenerated and decrypt drop refusing while a forced pass is owed, the forward waiver refusal, and ColumnMigration.load reading a missing schema as no history while rethrowing any other error. Also makes DirectionVocabulary readonly, makes forcedReason exhaustive, and corrects the PlaintextWriteError comment.
