io_uring SQE cancellation handling on peer socket disconnect

Summary

Replaced single user_data cancellation with cancel_requests_by_fd using AsyncCancel2 and CancelBuilder::fd(...).all() to idempotently cancel all pending socket operations upon peer disconnect.
