Summary

Stop a working active account from showing "re-login needed — refresh token dead" forever after Claude Code rotates its refresh token.

The deadlock

  1. Claude Code rotates the active account's refresh token during normal use. The live credential moves to a new lineage, but the slot backup still holds the consumed generation.
  2. A recovery POST of that backup returns invalid_grant, and the strike is bound to the backup's fingerprint.
  3. _entry_token_dead deliberately holds an active slot's strike while any stored source still matches the struck generation. The stale backup does.
  4. The only code that resyncs the backup to the rotated live credential, _resync_rotated_backup on the fetch fast path, never runs: the quarantine sentinel keeps the slot out of the fetch set.

The strike can never heal. The account keeps working in Claude Code, but ccswap list and status report it dead until ccswap add --slot N rewrites the backup by hand.

Observed on a real machine: struck fingerprint b9bd…, live credential 9a5e… (a newer lineage, in active use), backup still b9bd…. ccswap add --slot 2 cleared it at once.

Changes

  • New _heal_active_quarantine_from_live, called from the quarantine scan of _collect_usage_entries for the active slot only, before the dead verdict is computed.
  • It calls the existing _resync_rotated_backup only when the live credential is demonstrably a newer, usable lineage:
    • the strike is at threshold and bound to a fingerprint;
    • the live fingerprint differs from the struck one;
    • live has a full token pair and an unexpired access token;
    • the active read is neither degraded nor unreadable.
  • Ownership attribution (profile oracle), locking, verdict memoization and the never-raises contract all stay in _resync_rotated_backup. If it writes the backup, the existing clear_dead_token branch releases the strike, so no logic is duplicated.
  • _entry_token_dead is unchanged. test_active_strike_bound_to_backup_not_healed_by_live_fp still passes: a backup-bound strike still holds when nothing can prove the live lineage belongs to the slot.

Tests

New class TestActiveQuarantineHealsFromLive:

  • a rotated live credential attributed to the slot heals the strike: the backup now equals live, the sentinel clears and the strike row is cleared;
  • a foreign or unreachable probe leaves the strike and the backup untouched (parametrized);
  • an expired live access token, a degraded active read, or live on the struck generation attempts no probe and no resync.

uv run pytest -q -n auto: 4494 passed, 11 skipped, 1 xfailed.

Summary by CodeRabbit

  • Bug Fixes
    • Active accounts can now recover from a stale token quarantine when a valid, unexpired rotated credential is confirmed to belong to the account.
    • Quarantine remains in place when the credential is expired, the account data is degraded or unreadable, or the credential belongs elsewhere or cannot be attributed. This helps prevent access from being restored based on an unverified credential.

…ed live credential

The quarantine scan deliberately holds an active slot's strike while ANY
stored source (live or backup) still carries the struck generation. When
Claude Code rotated the refresh token, the live credential moved to a newer
lineage but the backup stayed on the struck one, and the only code that
resyncs it (_resync_rotated_backup, on the fetch fast path) never runs for
a quarantined slot because its sentinel keeps it out of the fetch set. The
strike could therefore never heal and a working account showed "re-login
needed" forever, until `ccswap add --slot N` rewrote the backup by hand.

Before declaring the active slot dead, the scan now attempts the existing
resync when live is demonstrably a newer, usable lineage (strike bound to a
fingerprint, live differs from it, full token pair, unexpired access token,
non-degraded and readable active read). Ownership attribution, locking and
memoization stay in _resync_rotated_backup; the existing clear_dead_token
branch then releases the strike.

Walkthrough

The active-account usage path now attempts to heal a dead-token quarantine from an eligible rotated live credential. Tests cover successful attribution, rejected attribution, unavailable attribution, and conditions that prevent resynchronization.

Changes: Active quarantine recovery

Layer / File(s) Summary
Active quarantine healing and validation
src/claude_swap/switcher.py, tests/test_switcher.py
The usage path attempts healing before its dead-token check. The helper requires a threshold-level strike with a bound fingerprint, a different live credential lineage with complete and unexpired OAuth tokens, and an active read that is neither degraded nor unreadable. Tests verify resynchronization and strike clearing when attribution identifies the account, and verify that other cases do not change the backup or trigger resynchronization.

Usage collection can be delayed repeatedly when ownership attribution is unavailable. The change is mergeable with owner awareness, but a cooldown and the negative-path test assertion should be added.

Explanation

The title names the main change: healing an active slot that was quarantined because of a stale backup, using the rotated live credential. The PR objectives describe the same change, where a threshold strike on an active slot is cleared through a resync from the live credential. The title is specific, short, and free of file lists or vague terms, so a teammate scanning history would understand the primary change.

Full details: Docstring Coverage

Explanation

Docstring coverage is 100.00% which is sufficient. The required threshold is 80.00%. Docstring coverage is scoped to functions touched by this diff with cyclomatic complexity of at least 2. Documentation-only edits and deletions within surviving functions are included; unchanged diff context and fully deleted functions are excluded. Analyzed 2 functions across 1 files. (1 skipped: 1 too large.)

Assert that ownership attribution was attempted.

The sentinel, backup, and strike assertions pass even when the eligible quarantine path skips attribution. Add probe.assert_called() to cover the foreign or unreachable attribution result.

Suggested fix
-        with patch("claude_swap.oauth.fetch_oauth_profile", return_value=profile):
+        with patch(
+            "claude_swap.oauth.fetch_oauth_profile", return_value=profile
+        ) as probe:
             entries = self._collect(s, info)

+ probe.assert_called() # attribution was attempted and rejected
         assert entries["1"].sentinel == USAGE_RELOGIN_REQUIRED

This is a distinct test-coverage improvement. Existing ownership tests assert probing on the normal fetch path, not on this quarantined collection path.

Add a bounded cooldown for failed or unverifiable ownership probes.

A quarantined active slot can call oauth.fetch_oauth_profile on every ordinary collection pass. The call is synchronous and allows five seconds. Removing the helper docstring’s “no network” claim would only correct the documentation; it would not bound the repeated delay.

Keep _probe_verdicts for definitive boolean ownership results. Add a separate timestamped cooldown, or extend the cache with an explicit expiring negative state. Do not store temporary failures as False, because False means a definitive foreign-credential verdict.
