fix: name the code a host reads on a configuration refusal, and say a blank persona id is refused - #687
Five docs pages told a host that a configuration refusal comes back "with CONFIGURATION_ERROR". That is the name of the enum member. The error's code is the string 'SYS_CONFIGURATION_ERROR', the value of GMIErrorCode.CONFIGURATION_ERROR in src/core/utils/errors.ts, so a host that compares err.code with 'CONFIGURATION_ERROR' never matches the refusal.
Each page now names the string a host compares:
docs/getting-started/SQL_STORAGE_QUICKSTART.md: the conversation manager's refusal of a conversations or conversation_messages table it did not make, whose message names conversationManagerConfig.tablePrefix.
docs/safety/PROVENANCE.md: AgentOS.initialize() refusing a provenance policy that leaves prefixed conversation tables unprotected.
docs/GMI.md: AgentOS.initialize() refusing a learning option that is not valid.
docs/AGENCIES.md: the full runtime refusing an agencies bound out of range.
docs/PERSONAS.md: AgentOS.create() refusing an inline personas list.
docs/GMI.md already wrote the member with its string beside it for a fallback failure, as CONFIGURATION_ERROR (SYS_CONFIGURATION_ERROR); that form stays allowed.
Six source comments the API reference is built from named the member alone as well: the TSDoc of ConversationManager.initialize(), GMIManager.createAgencySeat(), AgentOS's learning-option check, assertInlinePersonaDefinitions() and validatePersonaSource(), and a line comment in MetapromptExecutor. Each now names the member with its string. ConversationManager.initialize()'s TSDoc also names the option as its message does, conversationManagerConfig.tablePrefix.
docs/PERSONAS.md also said every refusal of an inline personas list names "the index or id". A value that is not an array is refused with the type it received in the error's details, and an entry with no usable id by its index alone; the page and assertInlinePersonaDefinitions()'s TSDoc now say which refusal names what.
An entry whose id is blank (empty or whitespace only, such as id: ' ') is refused by its index alone, but the page said its id is named "where that id is a non-empty string", and the refusal's message said the entry needs "a non-empty string id", which ' ' is. The message now reads personas[<index>] must be an object with a string `id` that is not blank., and the page and the TSDoc say blank where they said non-empty.
CI, test first. The first commit changes the quickstart assertion in src/api/__tests__/learning-store-and-scopes.e2e.test.ts, and its run failed at that assertion. The second adds src/core/utils/__tests__/configurationErrorDocs.test.ts, which reads every page under docs/ (the generated API reference left out) and fails on the member's name in backticks without the string beside it; its run failed naming the five pages. The commit that changes the pages turns both green. A third test commit pins, on each of the six pages that describe a configuration refusal, the sentence's words that name SYS_CONFIGURATION_ERROR, matches the member's name with or without backticks, and scans every comment line under src/ (tests left out); its run failed naming the six source comments, and the commit that changes them turns it green. A fourth pins the personas page's sentence on what each refusal names, and has agentos-personas-inline.test.ts expect each refusal's message and details through AgentOS.create() (a repeated id, an entry with no id, an entry whose id is a number, a value that is not an array); its run failed at the page's sentence, and the docs commit turns it green. The last test commit adds a blank id (' ') to those cases and pins the new message and the page's words; its run failed at the message and the page, and the commit that changes the message, the TSDoc and the page turns it green. The code behind each sentence is pinned by cases that expect code: 'SYS_CONFIGURATION_ERROR' (or GMIErrorCode.CONFIGURATION_ERROR): learning-store-and-scopes.e2e.test.ts, conversation-tables-provenance.e2e.test.ts, agencies-launch.package.e2e.test.ts and agentos-personas-inline.test.ts. The learning option's refusals had no case on their code; the case in agentos-learning.e2e.test.ts that refuses learning.namespace, learning beside a passed layer and an unknown mode now expects it.

Description
configurationErrorDocs.test.ts only searches for bare backticked CONFIGURATION_ERROR references
and asserts that none remain. If documentation removes the references entirely or uses wording
without the code, the test still passes because it never requires SYS_CONFIGURATION_ERROR to
appear.
The test's regex only matches bare backticked enum-member references, and the assertion checks that
this list is empty. Its separate discovery assertion only verifies that GMI.md exists; it does not
assert that any page contains SYS_CONFIGURATION_ERROR, even though the runtime enum defines that
as the value hosts read.
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution
## Issue description
The documentation regression test only rejects one incorrect form of the enum member and does not require any documentation to name `SYS_CONFIGURATION_ERROR`, so deleting all relevant code references would pass.
## Fix Focus Areas
- src/core/utils/__tests__/configurationErrorDocs.test.ts[32-44]
## Recommended Fix
Add positive assertions for the expected host-readable code and, preferably, for the targeted documentation pages or required paired forms. Keep the negative assertion for bare `CONFIGURATION_ERROR` references, but ensure the test fails when the required code is absent from the pages it is intended to protect.

**API docs still name the enum member**
When a host reads the public method's JSDoc instead of the quickstart, `ConversationManager.initialize()`'s public JSDoc calls the error `CONFIGURATION_ERROR`, while the error exposes `SYS_CONFIGURATION_ERROR` in `err.code`, so a host copying that name still misses the refusal.
Update the public JSDoc to name the `err.code` string and include source JSDoc in the regression check, or otherwise correct that documentation alongside the Markdown pages.

+- `tablePrefix` (`conversationManagerConfig.tablePrefix` on the runtime), unset by default, names the two tables `<prefix>conversations` and `<prefix>conversation_messages`, with their indexes. Set one where the database holds another component's table of either name: a memory Brain on the same adapter makes a `conversations` table of its own, with other columns, and so does `SqlStorageAdapter`. `initialize()` refuses a database whose table of either name it did not make, with an error whose `code` is `'SYS_CONFIGURATION_ERROR'` and whose message names `conversationManagerConfig.tablePrefix`, before it changes anything; `AgentOS.initialize()` fails with it. A prefix is letters, digits and underscores, starting with a letter or an underscore, at most 32 characters (`'agentos_'`, for one). Setting one on a database that holds conversations leaves those in the tables of the old name. A provenance policy names the tables it protects exactly: where it protects `conversations` or `conversation_messages` and not the prefixed name, `AgentOS.initialize()` refuses to start, naming the tables to add to `storagePolicy.protectedTables` ([Provenance](../safety/PROVENANCE.md#storage-policies)). Set the prefix before the runtime first starts on a database that will also hold a Brain: the check runs when the manager starts, so a Brain opened later beside the unprefixed tables finds a `conversations` table that is not its own. On PostgreSQL that Brain does not open (its `messages` table's foreign key names `conversations` columns the runtime's table lacks); on SQLite it opens, and its conversation export and import fail.
</code_context>
<issue_to_address>
**Conflict message names wrong option**
When a host hits a conversation-table conflict and relies on the message to identify the configuration field, `ConversationManager.initialize()` rethrows the table-conflict message unchanged, and its documented field name is `tablePrefix`, not `conversationManagerConfig.tablePrefix`; callers following the guide cannot find that full option path in the error message.
Describe the message as naming `tablePrefix`, and identify `conversationManagerConfig.tablePrefix` separately as the runtime config path.

**Incorrect refusal codes pass checks**
When a refusal page gives a wrong code, omits it, or uses an unbackticked wrong member, `BARE_MEMBER` only catches a backticked `CONFIGURATION_ERROR` without the expected value, and the enum assertion checks only the source; incorrect or missing `err.code` guidance therefore passes.
Assert each refusal page names `SYS_CONFIGURATION_ERROR` and reject wrong member names with or without backticks.

Assert the refusal code without pinning the sentence.

Correct the non-array validation wording.
When personas is not an array, assertInlinePersonaDefinitions reports the personas field and the received value. It does not identify an index or id. The sentence should limit “naming the index or id” to entry-level validation errors.

-`personas` replaces the file-system source: when it is set, no directory is read. @coderabbitai Two items from the reviews on aa33832 and 72ec957, which came in the review bodies rather than as threads:
docs/PERSONAS.md:68, "naming the index or id" (outside the diff range): fixed in 091ad31, test first in baf43c5. A value that is not an array is refused with no index or id (details.received holds the type), and an entry without a usable id is named by its index alone. The page now reads "The refusal of an entry names its index, and its id where the entry has one; the refusal of a value that is not an array gives the type it received in its details.", and assertInlinePersonaDefinitions()'s TSDoc says the same. agentos-personas-inline.test.ts now expects each refusal's message and details through AgentOS.create() (a repeated id, an entry with no id, a string) and pins the page's sentence; its run on baf43c5 (CI 41348741711) failed at the two page pins before the fix.
configurationErrorDocs.test.ts, pinning whole sentences (nitpick): the pins are now the clause that names the call and the code, such as "from AgentOS.initialize() with the code SYS_CONFIGURATION_ERROR", so "before any service starts" and the rest of each sentence can change without touching the test. The clause itself stays pinned: the point of the check is that each refusal page names the string beside the call that refuses, and a page that says only "the refusal" with the code elsewhere would not tell a host which call's error to compare.
@jddunn Both follow-ups address the review concerns.
Persona refusals:docs/PERSONAS.md and assertInlinePersonaDefinitions() now distinguish entry refusals from non-array refusals. The tests check messages and details for a repeated ID, an entry without an ID, and a string through AgentOS.create(). They also check that no provider initializes.
Documentation assertions:configurationErrorDocs.test.ts now pins refusal clauses rather than whole sentences. Keeping the refusing call and SYS_CONFIGURATION_ERROR together serves the test’s purpose. I withdraw the suggestion to loosen those clause assertions further.
I verified the current source and assertions. I did not run the tests or verify the reported historical CI result. No further change is needed for these two review items.

idis blank (empty or whitespace only, such asid: ' ') is refused by its index alone, but the page said its id is named "where that id is a non-empty string", and the refusal's message said the entry needs "a non-empty stringid", which' 'is. The message now readspersonas[<index>] must be an object with a string `id` that is not blank., and the page and the TSDoc say blank where they said non-empty.Linked issues

Type of change
How this was verified
CI, test first. The first commit changes the quickstart assertion in
src/api/__tests__/learning-store-and-scopes.e2e.test.ts, and its run failed at that assertion. The second addssrc/core/utils/__tests__/configurationErrorDocs.test.ts, which reads every page underdocs/(the generated API reference left out) and fails on the member's name in backticks without the string beside it; its run failed naming the five pages. The commit that changes the pages turns both green. A third test commit pins, on each of the six pages that describe a configuration refusal, the sentence's words that nameSYS_CONFIGURATION_ERROR, matches the member's name with or without backticks, and scans every comment line undersrc/(tests left out); its run failed naming the six source comments, and the commit that changes them turns it green. A fourth pins the personas page's sentence on what each refusal names, and hasagentos-personas-inline.test.tsexpect each refusal's message and details throughAgentOS.create()(a repeated id, an entry with no id, an entry whose id is a number, a value that is not an array); its run failed at the page's sentence, and the docs commit turns it green. The last test commit adds a blank id (' ') to those cases and pins the new message and the page's words; its run failed at the message and the page, and the commit that changes the message, the TSDoc and the page turns it green. The code behind each sentence is pinned by cases that expectcode: 'SYS_CONFIGURATION_ERROR'(orGMIErrorCode.CONFIGURATION_ERROR):learning-store-and-scopes.e2e.test.ts,conversation-tables-provenance.e2e.test.ts,agencies-launch.package.e2e.test.tsandagentos-personas-inline.test.ts. The learning option's refusals had no case on their code; the case inagentos-learning.e2e.test.tsthat refuseslearning.namespace, learningbeside a passed layer and an unknown mode now expects it.Migration notes
