Bump the version across the seven release files and cut the OpenCode v2 section of the changelog: version-aware OpenCode 2.0 public session transfer support (transfer, no-overwrite, dry-run), preserving portable tools, inline media, and completed compaction summaries.

Pós-merge de #1/#2/#3, o botoa de 'Unreleased' entra como
0.13.0 - 2026-10-17(MINOR: o OpenCode v2 é um formato novo). Bump nos 7 arquivos do handoff (pyproject.toml,__init__.py,uv.lock,tests/test_cli.py,docs/cli-reference.md,docs/specification.md,CHANGELOG.md). CI deve fechar verde.
