[codex] Rename public package to agent-memory-sdk

Summary

• Renames the public npm package from agent-memory / @agent-memory/sdk to the available unscoped package agent-memory-sdk.
• Updates workspace filters, publish/test workflows, docs, examples, and package-boundary tests.
• Keeps first-party @agent-memory/* packages private and bundled into the single public package.

Verification

Publish

• Force-moved tag v0.0.1 to commit 3a3b0b5 to trigger the Publish workflow for the renamed package.
• Publish workflow passed lint, build, test, pack dry-run, and npm publish.
• Confirmed on npm: agent-memory-sdk@0.0.1 with latest set to 0.0.1.
