API payment requests (format: json) triggered MissingTemplate inside the exceptions_app handler because ErrorsController only rendered HTML and only the 403 template existed. This masked the real exception behind a failsafe error. - Make ErrorsController#show format-aware: render JSON for json/any formats, HTML for html - Add missing 401/404/500 templates - Log the underlying exception (cause) with backtrace so the real root cause is no longer swallowed Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

Add explicit request logging to trace whether registry/registrar requests actually reach this billing instance and with what data: - ApplicationController: log every incoming API request (method, path, host, request_id, initiator, params) before authorization - InvoiceNumberGeneratorController: log generated number, configured range and actual db max - InvoiceGeneratorController: log create start/saved, and surface previously swallowed errors as logger.error + backtrace - OneoffController: log start (with found_in_db check) and outcome (ok/rejected) Diagnostic aid for the "invoice not found on oneoff" investigation.
