How to show hierarchical list of categories and pages?

Hello.

I have several categories and several documents in each category. I am hoping to have a display widget like so

• Category Name
  • Page in Category 1
  • Page in Category 1
• Category Name
  • Page in Category 2
  • Page in Category 2

I have tried to use both the [documents_categories] shortcode and the [documents_hierachry] short code to no avail. Either I get only a list of pages, or only a list of categories. There does not seem to be a way to combine the two.

Suggestions or thoughts? Thank you.
