Doc-only. The tier ladder gets its own section and table in the README, replacing the one-sentence version buried under the harvest table.

The tier ladder was one sentence buried under the harvest table. It is
now a section with a table: what each tier means and what passed to earn
it, plus the two boundary statements — the probe sets the tier, and a
higher tier means stronger evidence, not certainty.
