Goal

When a patch is ignored or becomes a noop, return exactly why, what operation was rejected, and the correct retry shape so LLMs never have to infer the fix from silence.

Current note

cannot remove swimlane through patch semantics; use swimlaneOps.remove is the standard to aim for, not bare noop: true.
