feat(adapters): add Ollama prompt reconstruction

Summary

• capture local Ollama /api/chat and /api/generate traffic through the shared explicit policy
• reconstruct system, messages, inline images, tools, options, JSON responses, and NDJSON streams into Prompt IR
• add exact NDJSON Raw byte ranges through Core, SQLCipher, Desktop API, fixtures/goldens, and progress tracking
