Deduplicate remote self-hosting azd templates

Remove dupliated remote self-hosting azd template files and reference the Azure-Samples azd template repos in the readme.

This PR removes duplicated Azure Developer CLI (azd) template files for remote self-hosting of the Azure MCP Server, consolidating them into external Azure-Samples repositories. The local README files are reduced to simple redirects to the corresponding Azure-Samples repositories.
