Manual refresh and reconnect forced a token refresh, which can return an empty account even while ordinary account reads and rate-limit requests remain authenticated. The toolbar interpreted that response as logout. Read snapshots without forcing token refresh for every refresh source, matching the working startup and timer behavior.
Bumps the app to 0.1.18 (18). The new regression reproduces the empty-account behavior and checks manual refresh plus reconnect. All 192 local tests pass. Three consecutive manual refreshes through the patched Swift store also passed against the local Codex account. Focused review found no additional actionable issues; existing explicit logout handling remains covered.
