docs: add AI contribution policy and PR disclosure checkbox#453

Adds an AI-assisted contributions section to CONTRIBUTING.md with iff conditions (own the code, meaningful tests, disclose use) and adds a matching AI Disclosure section to the PR template. closes #449 Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>
