bug: Codex MCP missing generate_skill tool — cannot generate Skill via origin=source in conversation

Describe the bug

The official docs describe three origins for Skill generation (experience, source, usage) and provide both CLI and HTTP API paths for all three. However, when using Codex conversation to generate a Skill with origin=source, the Agent reports that mcp__powercontext__generate_skill does not exist in the MCP tool registry. The only available generation path (create_dream_run / Dream Run) only accepts Experience Artifact references and does not support origin=source.

Steps to reproduce

1. Start Server with a Generation model configured.
2. Open a new Codex session, ensure PowerContext hooks are trusted.
3. Have a conversation that produces a Source (e.g., describe a deployment procedure).
4. Ask Codex to generate a Skill from the Source directly:
   plaintext
5. Generate a Skill candidate from the deployment Source we just discussed, using origin=source, skip Experience.
6. Observe the error: Codex reports mcp__powercontext__generate_skill is not available in the current tool registry.

Expected behavior

Codex MCP should expose a generate_skill tool (or equivalent) that supports all three origins documented in the official docs:

1. origin=experience — requires approved Experience Revision(s)
2. origin=source — requires precise Source reference(s), no Artifact references
3. origin=usage — requires target Skill Revision + usage Source

Actual behavior

1. mcp__powercontext__generate_skill is not available in the Codex MCP tool registry.
2. create_dream_run (Dream Run) only accepts Experience Artifact references; it cannot be used for origin=source.
