Rework spec headings - #602

This is extracted from the conformance v2 work. Previously the spec requirements were subheadings of conformance. This makes conformance an isolated section. The requirements is now a top level section which contains the bulk of the spec. Signed-off-by: Brandon Mitchell <git@bmitch.net>
