Materializes CDISC biomedical-concept classification data (schemes, values, and BC assignments from the cdisc-biomedical-concept-groupings service) into the existing concept_group/concept_group_concept tables, tagged as CDISC-sourced. Synced at app startup and via the BC Explorer refresh button, with a full idempotent replace-sync that never touches user-created Custom Concept Groups and survives the grouping service being temporarily unavailable. Because CDISC groups are just more rows in the existing tables, the whole assign/remove/display pipeline works for them unchanged. The "Add group" dropdown (concepts_cell.html, shared by the Activities page and SoA grid) now organizes options into scheme optgroups (COA Type, Collection Method, Concept Group, Implementation Domain Code, Age Category, Therapeutic Area) followed by Custom Concept Groups, and assigned-group headers show scheme context. The Custom Concept Groups admin page and its CRUD routes are scoped to user-created groups only. Consolidates three previously-duplicated "fetch concept groups for the dropdown" queries into one shared helper.
…e export Bumps the cdisc-biomedical-concept-groupings submodule to 854d5fc (black reformat + BC explorer prototype) and fixes build_usdm_bc_surrogates to exclude BiomedicalConceptSurrogate entries no longer linked to any activity.

Understand-Anything data is local tooling output, not project source; keep it on disk but stop versioning it now that .ua/ is in the global gitignore.
