The log files and the log database run every message through
CleanseLogMessage, but the console target didn't, and in Docker the
console is what `docker logs` shows: API keys in failing request URLs and
the PostgreSQL password in the migration log line appeared in clear. The
console layout is now wrapped in a ${cleanse} renderer. The colon inside
it has to be escaped, or NLog drops the rest of the layout.
Builds on Bookshelf pennydreadful/bookshelf#149, which cleansed the migration line alone.

resolves #101, use CleanseLogMessage when recording the migration info message. Also removes two unneeded readarr docs.
