Use the brand statusline on every forge's review comments - #13

All four forges now build inline comments from one helper: GitHub and GitLab render the brand status icons (rich markdown), Bitbucket and Azure DevOps get the identical severity/confidence/kind statusline in plain text since they display raw HTML. The silent clean-PR comment carries the pass icon on rich forges, and check summaries follow the same rich/plain split.

Unifies inline review comments across GitHub, GitLab, Bitbucket, and Azure DevOps behind one helper: rich-markdown forges (GitHub, GitLab) render the brand status icons, the others get the same severity/confidence/kind statusline in plain text because they display raw HTML literally. The silent clean-PR comment gains the pass icon on rich forges. Unit tests cover the rich and plain variants.
