The PR title describes output-flag refactoring, but the diff also silently changes the default model cascade from empty to three fallback models. This quadruples default LLM usage for the retry path and is not mentioned in the PR description, so it needs human confirmation before merge.

Confirm default cascade change belongs in this PR

The PR title/description only discuss output format flags, yet this line changes `Config::default`
to populate the model cascade with three fallback models
(`mistralai/mistral-small-3.2-24b-instruct`, `google/gemma-3-27b-it`, `qwen/qwen3-32b`) instead of
an empty roster. The e2e test at `tests/e2e.rs:865` now expects eight LLM requests (initial + repair
for each of four models) in the repair path, implying default API costs and latency increase
significantly. Confirm this behavioral change is intentional and should ship alongside the
output-format refactor, or split it into a separate PR so reviewers can evaluate the cost and
product impact independently.
