Diagnose: tests reference non-existent hello/hello world commands; actual commands are patch, commit, init
Diagnose existing bug: patch and commit commands use callback-style dir() without await, causing run() to return before async work finishes
Fix src/commands/patch/index.ts: wrap dir() callback in a Promise so run() properly awaits
Fix src/commands/commit/index.ts: same fix
Remove stale test/commands/hello/ test files
Add test/commands/patch/index.test.ts for the patch command
Add test/commands/commit/index.test.ts for the commit command
Add test/commands/init/index.test.ts for the init command
All 7 tests pass, lint clean, CodeQL clean
