Bumps to 0.4.0 and pins install docs to the v0.4.0 tag. Adds `verify` — the both-lane works-gate — plus doctor / tx / cookbook errors (P3 diagnose+verify surface), all live-verified on Hadrian.

Release v0.4.0. Adds the P3 diagnose + verify surface on top of v0.3.0:
