Improvements to escape routes (follow-up for #990) after testing it on cats 🐈

A parent with type arguments is in the class signature as well, and the note named the
signature rather than the parent, which is what a reader is looking for.

The note above the lines to paste was a `#` comment, which the report offers for
mimaBinaryIssueFilters too, where it does not compile. It is now `//`, and a filters file
takes either.

A note named the definition that exposes a class, which for a chain of private classes was
another one of them, leaving the reader to walk the rest. It now follows the chain to a
definition a client can name, folding steps that share a relation:

  foo.I1 escapes as a parent of trait foo.I2, trait foo.I3, object foo.Lib
