Issue body actions

There are some places in the code where we create an Issue without specifying the instance. We should specify the instance everywhere.

• 2x in extensions.py
• PK and FK checks in check.py (feat: ✨ check primary key exists #218 , feat: ✨ check foreign keys fields exist #227)
