feat(v2): add governed SourceOps lifecycle

Summary

Implements the governed RAPTOR v2 SourceOps lifecycle as five logical commits:

• V2 kickoff authority and implementation panel alignment
• V2-S1 SourceOps registry and deterministic offline validator
• V2-S2 staged snapshot verification and deterministic fact diffing
• V2-S3 materiality evaluation, proposal-only impact routing, and rollback rehearsal
• Mandatory RAPTOR WSL execution instruction

Safety boundary

This PR is additive and offline. It does not download, promote, activate, replace, delete, restore, schedule, dispatch, or execute any source or downstream action. Impact routes and rollback operations are proposal-only, require human approval, and have executed=false. It establishes no scientific, legal, clinical, worklist, submission, RescueScreen, or independent-panel authority.

Authoritative validation

All authoritative Python validation ran sequentially in WSL2 using:

/home/sdrona/raptor/bin/python (Python 3.12.3), PYTHONPATH=src

Latest integrated results on PR head 6c3e554ec3fa21a5eaa74bae5482c1938faa0825:
