Rename the remaining HUMMINGBIRD_PATIENT_ variables in .env.example - #113

The Nightingale rename swept app, config, routes, database, tests, scripts, bootstrap, and .github — but not the repository root, so .env.example still documented 44 variables under the old prefix that no code reads. Anyone provisioning a new environment from the template would have configured a patient product that stays fail-closed, with nothing to indicate why. Found while rebasing a July branch whose tests set hummingbird-patient.* config keys and failed against the renamed service. No code change: a repo-wide grep for HUMMINGBIRD_PATIENT_ across app, config, routes, database, tests, scripts, bootstrap, and .github is now empty. The failure mode is quiet: anyone provisioning a new environment from the
template configures a patient product that stays fail-closed, with nothing to
indicate why. The deployed host was fine only because its
.envwas migratedby hand during the deploy.

.claude/rules/parallel-agents.md:.env.exampleis a single-writerfile. This is a mechanical prefix rename with no semantic change, isolated to
its own PR so it can be reviewed as such.
