harness_adapter: clean stale limited bash markers - #71

Conversation
Drop active limited_bash markers when the run directory is already gone. For old markers with a live PID, compare the marker start time with the Linux process start time so PID reuse does not keep stale entries alive. Keep recent active markers with an existing run directory during a grace window, preserving parallel wrapped commands in PID-reused harness namespaces.
