docs: restore the Claude Code + Codex setup sections (missed by #15's squash) - #16

PR #15's squash landed only its first commit; the step-by-step setup sections for both agents (English and Vietnamese) did not make it onto main. This brings both READMEs to the intended full state — each with a per-agent Claude Code / Codex setup walkthrough and a pointer from the security section. PR #15 squash-merged only its first commit — the step-by-step "Setting up Claude Code and Codex" sections (English and Vietnamese) did not land on main. This restores them.
Each README now has a per-agent walkthrough: the exact install command, what pythia wires for that agent, where skills land, the by-hand steps the agent needs (restart; Codex
/mcp+/hookstrust), how to verify, and how a change gets approved. The security-section paragraph collapses into a pointer, and the compatibility table lists Codex.Docs-only; README lints pass.
