🔥 herdr の Claude plan mode 突入キーと claude-plan-mode.sh を削除する - #6

prefix+shift+m は claude-plan-mode.sh 経由で agent-base-repo の plan-effort-apply.sh を呼んでいたが、その hook は削除された。呼び先が無いと キーは何も起こさないため、キー定義、script、配置定義を取り除く。 Co-Authored-By: Claude Fable 5.1 <noreply@anthropic.com>

herdr の prefix+shift+m は
claude-plan-mode.sh経由で agent-base-repo の~/.claude/scripts/plan-effort-apply.shを呼び、model と effort を切り替えてから plan mode へ入れていた。この hook は u-ichi/agent-base-repo#107 で削除され、キーは何も起こさなくなったため取り除く。変更
