feat: rate limit detection and auto account rotation for Claude OAuth#64

Add errorType, limitResetAt, and accountKey fields to all handleCompletion mutations so the shared callback script can report rate limits. Task and session workflows auto-retry with the next available OAuth account when rate limited. Also includes quick-task and notification UI improvements.
