fix: target the exact ChatGPT effort control

Summary

• remove the fork-only generic neutral composer-pill fallback
• retain only the upstream slider-trigger and model-switcher selectors
• add a regression assertion that unrelated neutral menu pills cannot match
• bump the packaged runtime to 3.0.4
