Add Liveblocks vendor official adapter

• Adds Liveblocks as a vendor official platform adapter to the adapters directory
• Fixes getReadme to support GitHub monorepo subpaths (Liveblocks package lives at liveblocks/liveblocks/packages/liveblocks-chat-sdk-adapter)
• Updates SKILL.md community adapter list
